Refund & Returns Policy

All Form Five products are printed on demand, made to order specifically for you. Because of this, we do not accept returns or exchanges for change of mind, this is standard practice for made-to-order goods and is consistent with consumer law exemptions that apply to personalised and custom-manufactured products (including under EU Consumer Rights Directive Article 16(c)).

Damaged or defective items If your order arrives damaged or defective, we'll make it right. Contact us within 4 days of delivery with your order number and clear photos of the damage (including packaging). We'll assess the claim and offer a replacement or refund at our discretion. Claims submitted after this window or without photographic evidence may not be accepted.

Shipping damage vs print quality Prints can sometimes arrive with minor creases from transit, these typically resolve on their own or with light pressure. If the damage is significant or the print quality is genuinely defective, that's on us and we'll fix it, just reach out.

EU & UK customers As all products are manufactured to order, they are exempt from the EU/UK right of withdrawal under Article 16(c) of the EU Consumer Rights Directive. This exemption applies to goods made to a consumer's specification or clearly personalised. Standard consumer rights for faulty or misdescribed goods still apply in full.

Shipping Policy

All Form Five products are printed and shipped directly to and for you. Free shipping is included on all orders, no hidden fees.

Production & delivery times Orders are typically produced within 2–4 business days, then dispatched from a production facility local to your region. Estimated delivery times after dispatch (this is a guide only):

• Australia: 5–10 business days
• USA: 5–10 business days
• UK & Europe: 5–10 business days
• Canada & New Zealand: 7–14 business days

Customs & duties International orders (particularly UK and EU) may be subject to local customs duties or import taxes upon arrival. These charges are the responsibility of the customer and are not collected by Form Five at checkout.

Delays We are not responsible for delays caused by carriers, customs processing, or circumstances outside our control. If your order is significantly delayed, contact us and we'll look into it on your behalf.

Lost or undelivered orders If your order hasn't arrived within the estimated window, contact us with your order number. We'll investigate and arrange a replacement or refund if the item is confirmed lost.

Privacy Policy

We collect personal information (name, address, email, payment details) solely to process and fulfil your orders. This information is handled securely through Shopify's platform and shared only with the partners needed to deliver your order. We don't sell your data or use it for anything beyond running your order. You have the right to access, correct, or delete your data at any time this can be achieved by contacting us. Our products All products are printed on demand and made to order. Colours may vary slightly between screens and print, this is normal and not considered a defect (for more information google CMYK vs RGB). Product images are representative; minor variations in finish or texture may occur.

Pricing & availability Prices are listed in the currency of your market and include any applicable local taxes where required. We reserve the right to change prices or discontinue products at any time without notice. If a pricing error occurs on your order, we'll contact you before processing.

Intellectual property All artwork, designs, and content on this site are owned by Form Five and protected by copyright. You may not reproduce, copy, or use any content without our written permission.

Order cancellations Because products are made to order, we cannot guarantee cancellations once production has begun. If you need to cancel, contact us immediately and we'll do our best to help.

Liability We are not liable for indirect or consequential losses arising from the use of our products or site. Our liability is limited to the value of your order. Nothing in these terms affects your statutory rights as a consumer.

Governing law These terms are governed by the laws of Victoria, Australia. For EU and UK customers, your local consumer protection laws also apply and take precedence where relevant.
